** Plan benefit amounts reduce by 50% at age 65, regardless of age at enrollment.
*** This coverage includes a pre-existing condition limitation. If a claim is filed within six months of the policy's effective date, the claim will be reviewed to determine if it's due to a pre-existing condition. If the condition was present within the six months prior to the policy's effective date, no benefits will be paid. Upon six months after the effective date, a claim will no longer be reviewed for a pre-existing condition and would be eligible for claims payment.
